Position Summary
We are seeking a detail-oriented Registered Nurse (RN) to work from our office and support our Remote Patient Monitoring (RPM) program. The RN will review patient-generated health data, including blood pressure, pulse oximetry, heart rate, and other approved measurements.
This position is responsible for identifying abnormal or concerning trends, following established clinical protocols, contacting patient care teams when appropriate, documenting interventions, and escalating concerns to the supervising provider. The ideal candidate is comfortable working with electronic health records and technology-enabled care platforms.

Essential Responsibilities
- Review and interpret remote patient monitoring data in accordance with established clinical protocols.
- Monitor patient readings, trends, alerts, and compliance with prescribed monitoring schedules.
- Identify abnormal, missing, or potentially inaccurate readings.
- Triage patient concerns within the RN’s scope of practice and escalate urgent or clinically significant findings according to protocol.
- Notify the appropriate provider, physician, advanced practice provider, or care team when readings or symptoms require further evaluation.
- Respond promptly to high-priority alerts and document all actions taken.
- Maintain accurate, timely, and complete documentation in the electronic health record and RPM platform.
- Coordinate with providers, clinical staff, admissions personnel, and other departments as needed.
- Report recurring technical, clinical, or workflow issues to the appropriate supervisor.
- Protect patient privacy and maintain compliance with HIPAA and organizational confidentiality policies.
- Perform other related nursing and administrative duties as assigned.
- Current, unrestricted Registered Nurse license in Illinois or a multistate nursing license recognized in Illinois
- Graduation from an accredited nursing program.
- Experience using electronic health records and computer-based clinical systems.
- Strong clinical judgment, organization, and attention to detail.
- Excellent verbal and written communication skills.
- Ability to work independently while collaborating effectively with the care team.
- Ability to follow protocols consistently and recognize when escalation is necessary.
- Professional telephone manner
